AArtist Recognition
Mayor Warren and City Council to recognize Dee Marcellus Cole as the artist in the Artist Showcase (Community Services Coordinator Courtney Hodnick to present).
BGrant Acceptance on Behalf of Healthy Fontana
Mayor Warren and City Council on behalf of Healthy Fontana to accept a Kaiser Grant in the amount of  $25,000 (Greg Christian, Executive Director Kaiser Permanente Fontana and Ontario Medical Centers to present).

Approve the Revised Minutes of the April 9, 2013, Regular City Council Meeting.
BApprove Police Department 2012 Annual Report
Receive and accept the Police Department's 2012 Annual Report.
CBond Reduction for Tract No. 18824
Authorize a reduction in Street, Sewer, Storm Drain Improvement Bond No's. 03010400164 and 03010400165 posted by Van Daele Development Corporation for the backbone subdivision improvements for Tract No. 18824 located north of Duncan Canyon Road and west of Coyote Canyon Road from the combined original amount of $3,438,725.00 to a reduced amount of $859,681.00.
DPreannexation Agreement No. 13-001, Between the City of Fontana and FH II, LLC Investments for the Provision of Sewer Service to a 20-Unit Single-Family Residential Subdivision, Located on the North Side of Randall Avenue, West of Live Oak Avenue.
1. Find that the project is Exempt pursuant to Section No. 15268(b)(4) (Ministerial Project, Utility Connection) of the California Environmental Quality Act (CEQA) and, direct staff to file a Notice of Exemption; and,

2. Approve and authorize the City Manager to execute a Preannexation Agreement (No. 13-001) with FH II, LLC investments,  for the provision of sewer service to a 20-unit, single family residential subdivision located on the north side of Randall Avenue, approximately 170 feet west of Live Oak Avenue (Tract Map No. 18822).
Approve Consent Calendar Items as recommended by staff.
PUBLIC HEARINGS:
To speak on Public Hearing Items, fill out a card at the microphone stand indicating favor or opposition and give it to the City Clerk. Each person will be allowed 5 minutes to address the Council. If you challenge in court any action taken concerning a Public Hearing item, you may be limited to raising only those issues you or someone else raised at the Public Hearing described in this notice or in written correspondence delivered to the City at, or prior to, the Public Hearing.

AFiscal Year 2013-2014 One-Year Action Plan
Conduct a public hearing and adopt the Fiscal Year 2013-2014 One-Year Action Plan detailing the utilization of Federal Entitlement Funds in said Fiscal Year and authorize the City Manager to execute any and all documents necessary to effectuate said approvals.
